Referral form

Currently, we only accept referrals from school counsellors and social worker within Victoria, Saanich, and the Saanich Peninsula.

Please note that the information provided in this form will remain confidential. The information we collect is solely for the purpose of understanding the individual’s needs, facilitating contact with you, and ensuring a proper referral to a registered counsellor.

The Community is Medicine Counselling Fund will cover the cost of up to 5 counselling sessions for individuals in need. This support may be renewed for an additional 5 sessions, for a total of up to 10 sessions.

Please note that we are unable to accept every request. The purpose of this fund is to support youth who are falling through the cracks of the system. Priority will be given to young people who are experiencing complex trauma, struggling with addictions, or who do not have access to counselling services or other supports in the community.
